Does any one know what forms and actual copies of information are needed to Declare assets in the UK?

Looking for the same info for the States! Does anyone know if renal income in the States needs to be claimed as part of your Spanish income tax? I know I'll be charged for US taxes as well...can't imagine i'll have to pay both countries...
